I really liked the idea of the "also rans" campaign, which ends up with the PCs transcending a pre-ordained fate of mediocrity.
While not entirely Plane Above, there is inspiration to be found in the Living Gate lore, especially if combined with a strange twist in Erathis' Game of Making.
Both of those are on my official "ideas I'd like to examine more, if I had more time and energy" list.
